Hi All

Compared to what has gone before, the following Harvard occurrence is nothing spectacular and is very much par for the course for the type at the time and holds little from a research perspective. Nevertheless, if only for completeness' sake, this story needs to be related and documented.

No single type will be represented more than twice over a given 12-month period with exception of the Harvard for which a minimum/maximum of two Harvard occurrences will appear in every 12-month period guaranteed. Other types may be added in due course as legislation relaxes.

All summaries appearing in the database above are available for purchase by anyone at any time at R40 per summary all inclusive.

Simply let me know exactly what aircraft types you would potentially be interested in from the database and give me your e-mail, and I will contact you as soon as a summary involving that aircraft type becomes available for purchase.

Aircraft accidents and incidents are not to be confused. Technically, they are not synonymous as each term has its own defining limits and the two terms should never appear in the same reference to the same event.
So what then is an aircraft 'incident'?
It is any unexpected event (that did not form part of the original briefing) that has the potential to result in an accident and which results in NO damage to the military aircraft, state, public or private property and does NOT result in an injury to any person. To illustrate, losing a landing light on final approach is not an incident, but losing an engine on final approach definitely is! If the engine failure results in damage then it is an accident, but if the event results in no damage then it will be investigated under an 'incident'.
This month introduces a new and exciting development in my SAAF attrition - incident - as opposed to accident summaries.
Over the years I have collected substantial 'aircraft incidents' and have benefited greatly from the information contained therein. I have always wondered just how I might make this official information available to potential buyers who I know would find it equally satisfying?
The problem is that incidents are generally more benign and the degree of investigation usually less convoluted, making for a less detailed source document. For example, airframe hours, engine numbers and aircraft manufacture date which are par for the course in an accident Court/Board of Inquiry are seldom supplied even in officially complete incident reports. Drat!
While thus less complete or detailed compared with a Board/Court of Inquiry report, there are invariably some brilliant gems to be found amongst some of them. For example, while processing one such incident recently, the source document detailed the step-by-step technical procedure a specific engine underwent during its start up to the point of self sustainability. Fascinating stuff indeed!
Generally though, these official reports are much shorter and my summaries thereof inevitably equally so.
Since I did not desire to play around with my pricing structure too much and complicate matters, how was I going to provide a buyer with value for money for his purchase? The answer? Supply a package deal or dagwood of incident summaries! Yes, two or more incidents involving one aircraft type, which may or may not involve the same aircraft serial number. The package may include either ground and/or flying related incidents with period image accompaniment if available from the source document.
The only criterion of all the incidents I provide in a package is that each must be complete as to final results of their investigation ie, not just describing the incident but also the final findings of any investigation, defect report, 1022 action and recommendations etc.
Who would benefit from this kind of summary? Those who are already buyers of my accident summaries and who have a strong affinity for one or more specific aircraft types and who desire to want to know as much as possible about their beloved type.
Regarding the presentation below, I do not detail each incident separately since this would take up too much space.
This month's offering is thus the first package on offer and includes not one, not two, but three separate incidents, non of which are period image accompanied ie three incidents for the price of one! Hi All

I always enjoy the privilege of introducing a new aircraft type to my accident archive and this is no exception.

You can thank Covid-19 for the appearance of this accident summary at this time, for without this curse, it might never have seen the light of day!

After having caught up with everything I needed to during the first week of the national lock down, I thought what I might do to pass the lingering time ahead productively? This was just the time to attempt to process the longest and most intimidating accident yet to be compiled by me and one I had put off many times before.

Enter the triple HS.125 tragedy of 1971.

Even while taking down the source information from Archives in 2014, I decided that when I finally got down to this accident it was going to be so much more than just the accident itself. It was also going to be a serious consideration of the RSA 10th Anniversary Festival of which it inevitably became an unwelcome part.

So while James Byrom's fine publication WINGS OF AIR covers the accident accurately and in some detail, I obviously take this much further and head deeper into all aspects of the accident itself, but without leaving out any SAAF related detail as to the Festival. Included, for example, is the exact breakdown of all the approximate 200 Air Force aircraft that took part, from what bases they each operated, how many aircraft of each type, what formations were flown by each type, holding altitudes prior to run-in and what time they were to overfly their 'target', the challenges that some aircraft faced regarding the weather, the navy and air force search and rescue aspects detailed etc. And, yes, the serials of some of the aircraft involved were also officially mentioned and have been included for completeness.

So, If you are interested in a detailed record of the events leading up to this accident and the investigation that followed, short of the intimidatingly thick source document itself, this is as good as its going to get. For your information it took me an entire day at Archives to dictate the entire source document onto a tape recorder.... (pedantic yes, I kept record!)

I have also included all the images from the Board of Inquiry report including a topographical map showing the actual and scheduled flight paths and images of the two trees struck and all three initial impact points and direction of flight.

Our next consideration will be another Shackleton offering.

Hi All

This month's offering is nothing particularly spectacular but an inevitable consequence of extended low-level operations over either land or sea.

That said, there are some aircraft types that are just plain interesting, no matter how benign the occurrence and the graceful old Shack sure does it for me.

The next accident summary will be yet another new type, this time in the rotary line. If you apply a process of elimination you can work it out.

Hi All

It would be appropriate to start the new year with a new aircraft type attrition event, if only by sheer co-incidence. With two new types introduced in such short successsion, you can be certain that it will be a while before another new type is even considered for release by me.

So, here follows the first SA 330 Puma occurrence dating from a very early service period.

So how many crew members comprised initial SAAF Pumas operations?

The next occurrence will involve the Dakota.

Summaries vary in length from about ¾ page of text to as many as 12 excluding images and image captions.
Note that not all occurrences are image accompanied depending on the circumstances of the occurrence and what copy of the Court/Board of Inquiry/Station Investigation I get to process.

Accidents will cover the time period 01/10/42 to 31/12/75 as per the high-end limitations imposed by current legislation in terms of the Promotion of Access to Information Act and involve all types of attrition, both benign and spectacular, to exclusively the following types: the Harvard/Texan, C-47/Dakota, C-54/Skymaster, C-130 Hercules, Transall C-160Z, Merlin Mk IVA, HS.125 Mercurius, Viscount, Shackleton, Piaggio P.166S Albatross, Cessna 185, AM-3CM Bosbok, two Do 27, one Auster, one Spitfire, Canberra, Buccaneer, Mirage III, Vampire, Sabre, Impala, Wasp, Alouette II and III, Puma and the Super Frelon. No single type will be represented more than twice over a given 12-month period with exception of the Harvard for which a minimum/maximum of two Harvard occurrences will appear in every 12-month period guaranteed. Other types may be added in due course as legislation relaxes.

All summaries appearing in the database above are available for purchase by anyone at any time at R45 per summary all inclusive.

Simply let me know exactly what aircraft types you would potentially be interested in from the database and give me your e-mail, and I will contact you as soon as a summary involving that aircraft type becomes available for purchase.

Although nothing spectacular, this summary just had to be written for completeness.

Although complete in every way, the strong point again is the period images which are ever revealing and with unit badge prominently shown.

Our next consideration will be another faithful Harvard offering.

Hi All

This was another challenging summary to have to try and weld together successfully and I'm not fully certain that I achieved that aim perfectly.

Death of personnel is always tragic, but it is, I think, especially so when it results from factors beyond human control.

Our next consideration will involve the quaint Alouette II.

Hi All

Vortex ring state has claimed quite a few damaged and destroyed SAAF helicopters over the years and this summary is no exception.

Our next consideration will involve the Canberra.

Hi All

Whilst flying in support of SA Army forces training in anti-guerilla tactics in the Cedarberg Mountains, this helo was involved in an inadvertent encounter with an invisible ground threat and seriously damaged accordingly.

The next consideration will involve the Dak, which I have decided to double up on due to the number of occurrences from this type, to two per year as with the Harvard.

Herewith, the promised C-47 Dakota accident detailed below.

May I repeat at this point that some prospective overseas buyers of my Accident Summaries have indicated that the bank charges for making payment via our banks is horrendous. As a result, one buyer proposed whether he could pay me in cellphone airtime instead? Brilliant idea! He simply forwarded the link to me and a minute later payment was confirmed. All summaries appearing in the database above are available for purchase by anyone at any time at R45 per summary all inclusive. This may be adjusted upwards depending on the number of images accompanying the main text at my discretion.

Hi All

This just happens to be the earliest period Sabre accident I have had the honour of covering to date.

This tragic avoidable accident was, unfortunately, one that was all too common among ground attack aircraft types down through time.

Sadly, at least one period image had been STOLEN from the source document and so we are all the poorer therefore. Fortunately, my God misses nothing and justice WILL be served in due course on the individual concerned should the missing image not be restored to its place.

Unfortunately, this is an all too familiar occurrence in many accident reports and although I did recommend an easily applicable solution to the problem to the director of department at that time, I know not whether this was subsequently applied.

The next offering will involve a lesser known Vampire mid-air accident.

Since this is my first ever consideration of an incident involving air-to-air firing, I was eager to see what I might learn about these practices during the 1950s. This accident summary doesn't disappoint in this regard.

Vampire pilots flying air-to-air attacks against towed targets had to adapt themselves depending on the tow aircraft being flown against whether this might be towed by another Vampire, a Harvard or a Sabre. Naturally, the speeds of these tow aircraft varied considerably and had to be compensated for by the pilot during any attack. Over-eagerness and inexperience inevitably claimed some victims.

This accident report details the different target scenarios and interestingly, also gives the experience from the tow aircraft pilot's perspective and how he dealt with the consequences.

The next offering will involve a no doubt unknown incident involving the Super Frelon helicopter.

Summaries vary in length from about ¾ page of text to as many as 12 excluding images and image captions.
Note that not all occurrences are image accompanied depending on the circumstances of the occurrence and what copy of the Court/Board of Inquiry/Station Investigation I get to process.

Accidents will cover the time period 01/10/42 to 31/12/75 as per the high-end limitations imposed by current legislation in terms of the Promotion of Access to Information Act and involve all types of attrition, both benign and spectacular, to exclusively the following types: the Harvard/Texan, C-47/Dakota, C-54/Skymaster, C-130 Hercules, Transall C-160Z, Merlin Mk IVA, HS.125 Mercurius, Viscount, Shackleton, Piaggio P.166S Albatross, Cessna 185, AM-3CM Bosbok, two Do 27, one Auster, one Spitfire, Canberra, Buccaneer, Mirage III, Vampire, Sabre, Impala, Wasp, Alouette II and III, Puma and the Super Frelon. No single type will be represented more than twice over a given 12-month period. Other types may be added in due course as legislation relaxes.

All summaries appearing in the database above are available for purchase by anyone at any time at R45 per summary all inclusive. This may be adjusted upwards depending on the number of images accompanying the main text at my discretion.

Simply let me know exactly what aircraft types you would potentially be interested in from the database and give me your e-mail, and I will contact you as soon as a summary involving that aircraft type becomes available for purchase.

Although this system is prone to abuse, I would strongly advise that individuals would refrain from doing so, since it may well compromise the success of this effort and if I feel that it is not being sustainably successful, I will have no choice but to discontinue this effort and the future of summaries being available to the public will be forever jeopardised. By personally honouring the above terms, you will thus be investing in the future of more summaries being made available, both for your personal information and for that of others.

Hi All

It is unlikely that this accident made the media at the time, which makes this summary all the more significant.

I have previously employed an accident summary 'sandwich' for accidents/incidents that are too brief in length to justify being included individually for someone purchasing.

Once again, I have here resorted to a sandwich of two disparate Super Frelon events: one accident and one incident.

Both accounts are fully detailed and in every way complete.

I can assure you that, although the accounts are shorter than a Court/Board of Inquiry summary, they are by no means any less fascinating and sometimes include details you simply won't get elsewhere!

For those that have developed a healthy appetite, my next consideration will be a further DC-4/C-54 'sandwich'.
