Hi All

My first ever attrition occurrence emanating from Port Elizabeth is the source of this month's event.

Although the attrition to the SAAF's Sabre jet doesn't quite inspire me as some other SAAF fast jets, this one, more than any other to date, would appear to buck the trend.

This summary is particularly noteworthy for its excellent quality period image accompaniment. It also identifies both the pilots and aircraft serials of at least five of the nine Sabres that flew that day. This aside, it does not offer much from a research point of view, besides probably being the first safe ejection ever over SA soil and the first from a Sabre over the RSA!

For those familiar with my work, you will know that I always include airframe hours at occurrence, where this has been supplied. Sadly, many of the 1940s and even '50s source documents fail to include it as (irrelevant?). I beg to differ.

Although it may not be relevant to the majority of accidents, it is a nice-to-have if it is available. I find it particularly interesting to see at what airframe utilisation a particular accident event took place. The more events, the more interesting and relevant this figure becomes. If compiling a chronological attrition history for a particular aircraft serial, it is definitely a plus to have for every attrition event. It is but a single petal that adds overall completeness to any single attrition event and enables an attrition history to 'bloom' more fully.

The next release, probably in March, will involve the Cessna 185.

01/01/2018: Sabre 379* collided in mid-air with Sabre 354 during the practise display for Port Elizabeth's centenary celebrations on July 15, 1960 (3.5 pages text plus 16 period images);

No single type will be represented more than twice over a given 12-month period with exception of the Harvard for which a minimum/maximum of two Harvard occurrences will appear in every 12-month period guaranteed. Other types may be added in due course as legislation relaxes.

All summaries appearing in the database above are available for purchase by anyone at any time at R40 per summary all inclusive.

After a two month break, this month's offering is nothing exceptional, but covers all the bases for a complete accident, and a serious one at that, especially if you are in to the 'beastly' C.185. The images are quite revealing too.

I have also included the full costing table of the aircraft and major components when new and with depreciation at the time of the accident.

This accident offers almost nothing from a research perspective, but, that said, I would not have left it out for the world!

The next release, most likely in April, will involve the iconic Dakota.

Like the Buc, Shack, Wasp and C-130, the Dakota mostly produces really great accident summaries and this one is no exception. This is also my third write-off in succession, not bad considering that I only did one during the whole of 2017! But this is sheer coincidence.

For me, this accident had even greater significance since (albeit many decades later!) I walked around and possibly even occupied some of the same buildings alluded to in this summary, at what was then SLT/SLO at Lyttleton, during my initial SAAF training. I also had the privilege of having numerous both piston and turbine powered Daks fly quite low overhead our billets during their circuits from Swartkop. This undoubtedly helped me foster my passion for the old bird, though I must admit to preferring piston over turbine! If offered a 1 hour flight in a Dak or a SR-71, I'll take the Dak thank you, preferably all the way from Durban to Ysters at lowish level along the coast with ear muffs? Nice. There are not too many people who would appreciate such a flight as I would. I have never flown in the piston Dak before but only the turbine Dak. I love most SAAF aircraft, but I do love some more than others.

Unfortunately, there were no period images accompanying the original source document from Archives that I accessed. Just as well then that many years ago, someone kindly donated to me an image that his father had taken of him as a very young boy standing in front of the crashed aircraft, but in no way hiding any detail. In fact you only make out the lad a few moments later. It is quite a good, sharp pic at that. This revealing private period image now forms part of my official summary! I just knew it might come in handy some day when I finally did this accident, but I could not then have imagined just how handy it would be. Thank you. I still hope to come across some official images at some point down the line....

The next release will be a completely new aircraft type and the only new type release for 2018. It will remain a surprise until then. Might it be one of your favourites?

Hi All

If my descriptions under the 'updates' section immediately below sound very vague and non-specific then know that it is intentionally so. For full details, you obviously need to get the summary.

As promised, a completely new aircraft type this time around – Piaggio's P.166S Albatross!

Despite this type's shortcomings, there is just something about this portly machine that simply endears one to it.

This summary is nothing exceptional, but it is complete in the fullest extent possible and a fine record of this serial's possibly first ever attrition event?

What makes this occurrence more interesting is the fact that one of the pilots was later killed in the only fatal accident to befall this type in 1976.

The next summary will be only my second C-130 Hercules accident to date and quite an unusual accident this one is too.

Hi All

Only my second Hercules accident summary, the Defence Force's early inexperience in operating large transport aircraft is apparent in this unusual accident, although the BOI officially made the error of calling it an 'incident'.

There is little to be gained from a research perspective except possibly the aircraft manufacture date.

The next summary will be another Alouette II event.

No single type will be represented more than twice over a given 12-month period with exception of the Harvard for which a minimum/maximum of two Harvard occurrences will appear in every 12-month period guaranteed. Other types may be added in due course as legislation relaxes.

All summaries appearing in the database above are available for purchase by anyone at any time at R40 per summary all inclusive.

Hi All

This release is typical of this aircraft type. It is unspectacular and holds nothing from a research point of view. That said, it crosses all the Ts and dots all the Is and would be indispensable for anyone wanting to compile a detailed attrition history of this particular serial.

In my opinion the photographer went a bit 'ape' with the shutter!

The next summary will be another fine Harvard occurrence.

Hi All

Of all the Harvard attrition occurrences covered here to date, this one's serial is the highest of any by far.

For the researcher interested in Harvard history and the ground and air units that formed part of this aircraft's development during the WWII era, and in fact, anyone interested in compiling a definitive history of Brooklyn Air Station (BAS), this accident summary is a must have.

Although the accident itself is nothing spectacular, and technical in parts, two facts emerge from this summary that I could not recall ever having read or heard of before – two 'new' units at BAS. So surprised was I on hearing of them that I immediately launched an effort to read both Dave Becker's YELLOW WINGS and Dave and Winston's AT-6 HARVARD IN SA SERVICE to search them out for some mention of these units. Alas, there was none!

Knowing Dave as I did, I just know that he would never have left out something as significant as these two units from the two most appropriate books for them. As unbelievable as it sounds, clearly, even the great Dave appears not to have had any knowledge of the units in question. And you can be sure that he would have wanted to know about them.

The location to which this aircraft was being ferried during the accident flight also corroborates a statement made in the Harvard book.

I thus dedicate this summary to his memory.

The primary cause is not one uncommon to the type and indeed, we have covered it on numerous previous occasions here.

The next summary will be an, until now, very unique causation involving the Alouette III.

All summaries appearing in the database above are available for purchase by anyone at any time at R40 per summary all inclusive.

For the pedantic among us, this is the first reconsideration of the same aircraft serial for this type (see large list below).

It is always refreshing to have a new causation to consider. Every accident we have covered of the Alouette III to date has been so entirely different from the previous and once again, here we have no exception. In fact, this accident involves members of the then South African Police undergoing training with the SAAF.

There are no supporting period images in this instance.

This accident offers very little from a research perspective.

The next accident will be only our second ever of the Canberra and my first ever “weapon accident.” Warning - It is a goodie!

Hi All

Many will be familiar with this uncommon accident, but why did it happen and who was culpable?

I made a special point of including the scientific reasons, appearing in the original report, as to how the ingestion was physically possible given the size and weight of the ingested subject. Numerous diagrams clarify the text while period images illustrate the engine air intake and engine damage sustained.

Given the unusual nature of this accident, I also included greater detail than usual from the autopsy report.

Our next consideration will be another unusual accident but involving two disparate types, a jet and a prop plane.

Hi All

This accident unusually involves two disparate SAAF aircraft types, one Harvard and one Impala in an unlikely meeting that resulted in twisted metal.

This is my most recent Harvard consideration to date and can be considered an exception in this regard.

This occurrence offers little from a research perspective though the images are quite revealing.

Our next consideration will be another Super Frelon occurrence with a difference.

Hi All

This is our third Super Frelon accident occurrence consideration and all involving the same serial (tail) number.

There is little research value obtainable from this accident, but it's simply an occurrence with a difference although I am also aware of another Super Frelon accident in which rotor downwash on landing blew stacked galvanised iron roof panels onto private property causing damage, if I recall correctly.

A very abbreviated version of this occurrence was reproduced in the December 2018 issue of World Airnews aviation magazine.

Three period images (as printed in the magazine), besides being fine studies of a landing S/Frelon, show the event as it occurred in real time.

Our next consideration will be a truly fascinating and informative Mirage III accident and my vote for probably the attrition occurrence of the year.

Although the next Accident Summary release was to have been today, I have taken the difficult decision to delay it by one further month. This is necessary in order to permanently dispense with what would otherwise be a recurring logistical issue for me.

Better late than never....

I thoroughly enjoyed the compilation of this particular Incident Summary which could so easily have ended very differently for the aircraft and pilot involved.

This occurrence gave useful insights regarding how much fuel is used during taxi to the runway and facts and practices relating to the Mirage III's fuel system and drop tanks in particular, which I found fascinating.

Our next consideration will be another 'Spammy' indulgence.

How would you enjoy your personal copy of my Accident Summaries? Better still, how would you like to receive them in your inbox in original full length version with full supporting period and other image accompaniment with relevant tables/charts/diagrams/transcripts etc where appropriate!? Now it is possible. With Dean's consent, commencing about the start of each month and every month thereafter, I will announce the availability of one previously unpublished Accident Summary here.

Summaries vary in length from about ¾ page of text to as many as 12 excluding images and image captions.
Note that not all occurrences are image accompanied depending on the circumstances of the occurrence and what copy of the Court/Board of Inquiry/Station Investigation I get to process.

Accidents will cover the time period 01/10/42 to 31/12/75 as per the high-end limitations imposed by current legislation in terms of the Promotion of Access to Information Act and involve all types of attrition, both benign and spectacular, to exclusively the following types: the Harvard/Texan, C-47/Dakota, C-54/Skymaster, C-130 Hercules, Transall C-160Z, Merlin Mk IVA, HS.125 Mercurius, Viscount, Shackleton, Piaggio P.166S Albatross, Cessna 185, AM-3CM Bosbok, two Do 27, one Auster, one Spitfire, Canberra, Buccaneer, Mirage III, Vampire, Sabre, Impala, Wasp, Alouette II and III, Puma and the Super Frelon. No single type will be represented more than twice over a given 12-month period with exception of the Harvard for which a minimum/maximum of two Harvard occurrences will appear in every 12-month period guaranteed. Other types may be added in due course as legislation relaxes.

All summaries appearing in the database above are available for purchase by anyone at any time at R40 per summary all inclusive.

Simply let me know exactly what aircraft types you would potentially be interested in from the database and give me your e-mail, and I will contact you as soon as a summary involving that aircraft type becomes available for purchase.

Although this system is prone to abuse, I would strongly advise that individuals would refrain from doing so, since it may well compromise the success of this effort and if I feel that it is not being sustainably successful, I will have no choice but to discontinue this effort and the future of summaries being available to the public will be forever jeopardised. By personally honouring the above terms, you will thus be investing in the future of more summaries being made available, both for your personal information and for that of others.

Note: Due to logistical limitations on my part, payments made in a given month may only see that summary e-mailed during the first week of the following month.
