Looks like an 81-HB-62 81mm illumination mortar. It's supposed to come down with a parachute, so I have no idea what chain of events went wrong for it to not only be fired over a residential area but to come down ballistically as well.

some one is in the deep trouble.

Shocked she is not dead, I mean a mortar is heavy.

It may not have been a direct hit, she may have been struck by debris from the roof.

Seems two members of the Wits Rifles were to blame. After an event at the base in Dalview, an inventory check was conducted and one illumination mortar was found to be missing. After questioning the soldiers, two of them apparently admitted to there having been an 'explosion' at 11am. Further details weren't given. Those two are going to be in deep trouble over this, especially as they seemingly didn't report it when it happened.
